Care Management and Social Services Aultman Orrville Hospital

The Care Management Department consists of a Registered Nurse and a Licensed Social Worker who work together to assure that your hospital stay and transition to the community or other care facilities are appropriate to meet your needs. We provide support to you and your family in the areas of lifestyle transition, planning for future therapy and for care after your stay with us. We will assist you with your social needs during your stay, develop a comprehensive discharge plan and make referrals to appropriate community agencies to assist you with your return to the community.
