Your Rights As A Patient
- Patients have the right to skillful, courteous and hospitable care in a safe environment.
- Patients have the right to admission for treatment and care regardless of their race, color, creed, nationality, religion, or the nature of the source of payment.
- Patients have the right to receive information regarding their diagnosis, treatment and prognosis.
- Patients have the right to expect privacy and confidentiality.
- Patients have the right to know by name and position the persons caring for them.
- Patients have the right to share in their plan of care through discussion with physicians and nursing staff.
- Non-English speaking, blind or deaf patients have the right to request assistance (interpretation). We will make every reasonable effort to provide assistance.
- Patients have the right to freedom from restraints used in provision of acute medical and surgical care unless clinically necessary.
- Behavior management patients have the right to freedom from seclusion and restraints used in behavior management unless clinically necessary.
- Patients have the right to examine a copy of and request assistance in understanding their hospital bill.
- Patients who have questions or concerns regarding their rights may contact the Nurse Manager or Risk Manager.
- Patients have the right to formulate advance directives; however, appropriate care will not be withheld as a result.
- Patients have the right to appropriate assessment and management of pain.
Responsibilities of You and Your Family To Ensure Safe Care
- To report any perceived risks in your care.
- To provide accurate and complete information to doctors and hospital staff regarding health status.
- To follow the treatment plan developed with the doctor and other hospital staff.
- To express any concern regarding ability to follow the treatment plan.
- To ask questions if instructions or information provided are not understood.
- To discuss and understand the consequences should a decision be made not to follow the recommended treatment plan.
- To provide notice if unable to keep an appointment.
- To follow the rules and regulations of the hospital.
- To be considerate of the rights of other patients, visitors, and hospital employees.
- To be considerate of the property of others.
- To keep care givers informed of their level and relief of pain.
Note: In circumstances where the patient is a child or minor, these rights and responsibilities are those of the parent or legal guardian.
